District required Meal Disclosure Forms

Are you eligible for fee SAT/ACT waivers? IMPORTANT INFORMATION!

Low-Income? Think you may be eligible for fee waivers? Here's what you must do first:

Everyone eats fo
r free at Nova High. This does not automatically qualify students for fee waiver!

Before we may issue any type of fee waiver (testing, college applications, collegiate athletic eligibility), the parent/caretaker must complete the district’s 2 required forms authorizing the school to issue fee waiver(s) to the student. These forms comply with federal laws, and must be completed 8/1 of each school year, even if they were completed the previous year. Fee waiver-eligible students may register up to the late registration deadline for the SAT without late fees, but will be required to pay the late fee for late ACT registration.

Your parent’s/caretaker’s email address must be the same as what is on record with the school for this to work. If it is not, please contact Ms. Benjamin at ideline.benjamin@browardschools.com or 754-323-1672 for instructions on having it added.

Parent/Caretaker must follow the instructions below to complete the forms:

It can take up to 48-hours for approval to appear in the school's database.

1. Go to https://www.myschoolapps.com, select "Broward County Public Schools Household Income Survey 2023-2024" in the county drop down menu. The student will either be approved or denied.

2. After the parent/caretaker completes the Survey, s/he will receive a reference number and must click” yes,” when asked if they are done.

3. The parent/caretaker will then be directed to the Meal Disclosure Form at https://www.browardschools.com/food-nutrition. Select the option to be considered for all benefits. FYI: Most issues with eligibility happen because the parent/guardian does NOT complete the Meal Benefits Disclosure Form.

It can take up to 48 hours for the district’s database to update with the application information and the student’s name to appear there. We will then be authorized by the parent/caretaker to issue the waiver(s).  There are no exceptions to this requirement, which is in compliance with federal guidelines. Before we are permitted to issue any type of fee waiver (testing, college applications, collegiate athletic eligibility), the parent/caretaker must complete the district’s 2 required forms authorizing the school to issue fee waiver(s) to the student. These forms must be completed every school year, even if they were completed the previous year.

  • IMPORTANT: To take full advantage of an SAT or ACT test registration fee waiver, you must follow through and take the test on your registered test date.

Both online applications have a series of questions that you will answer to determine your eligibility to waive application fees. Your school counselor will verify your waiver eligibility.

  • DO NOT PAY TO APPLY TO COLLEGE! On the payment page, select "pay with fee waiver." If this is not an option, "trick" the form and indicate you are paying with a check or money order. Doing this will tell the school that they'll be receiving something from you in the mail!

  • Fill out and sign your Application Fee Waiver Request Form (you may use either a College Board or ACT application waiver request).

  • Mail, upload or email the completed waiver to the college/university Office of Undergraduate Admissions.
